Navigate to the Equipment Request Form

site, navigate to the Information Technology Page or click on the direct link provided by your IT Department. - Once on the IT page, click on Equipment Request Form located on the left, under "Information Technology Support."

Fill in Requester Details
- Complete the "Requester Name," "Requester's Email Address," "Requester Contact Number," and "Requester Title" fields.
- Note: Questions may vary based on your selected title.

Specify Request Type (On Behalf of Someone Else)

- Check the "Yes" box for "Is this request on behalf of someone else?".
- Select the "Recipient" type (Sister or Staff).
